I would like to know if anyone has ever ported this to a module?

Also does anyone have any experiences using this program?

I am thinking that phpFormGenerator could very well be used as a stand alone, which it is, and then add the form to a module........

.... any thoughts?

I am looking for the same functionality to control mischive makers. Hopefully someone will have an answer here somewhere.
