this link works but won't display anything within the theme.

It should be "file=addentry" only, not "file=addentry.php"

This is the actual example. I read another post about not using the name field in the form. Once i renamed the name field i was able to fix some of the problems i was having. However this still won't display using this link. If you go to the site and click guest book, then click on one of the subsaquent pages you will see how it won't disply within the theme.

Simple enough, you don't need the file=index at all. With no specified file, it will assume index. And second, your url has two question marks

Try http://www.delmarvanightlife.com/modules.php?name=gbook&start=1
