Need a modified new user form

I need somebody to set up a form for new users that requests more than what is set by default. I need to have something like this: http://www.globalgamingcenter.com/bettersignup.htm.

I also need people to be able to see those extra fields when they view the user's profile. The user also needs to have a way of editing what he chose afterward.

Can somebody do this for me?
