ONCLICK link 2 specific file in module or embed 2 files to 1

I've got 2 modules that I want to combine into one. They both work fine separately. The main one is the module that I created. It has a submit button on it that points toward the second module. The second module is simply a form (extremely modified clan recruitment form, not used for clan recruitment)

The form of course is on the index.php file in the form module. I simply either want to integrate it by changing the name of this file to say..."submit.php" or I would like to add the code to it inside the index.php file of my own module (this option would make it more streamlined wouldn't it?

If I do the first option, I would simply need to change this code...

to be directed directly to the submit.php that I renamed and moved to my module. How can I do that?
I tried changing it to this code...

but of course it doesn't let you access that file directly

If I embed the code in the submit.php file into the code of my index.php file (simply to cut down on numbers of files) is this doable?
